Back to School Health Fair at Clive Community Services

Saturday, August 4th

8:30 AM to 12:00 PM

Clinic registration 8:30-11:30

Come get your school and sports physicals, vision screenings, immunizations*, haircuts, school information/assistance with forms, and other community resources! Services will be provided free of charge on a first come, first served basis.

*Immunizations will be provided by Polk County Health for those who are under/uninsured, Medicaid-enrolled, American Indian or Alaskan Native. Please bring vaccination records if available.

Vision screenings will be offered by Clive Lions Club. School physicals will be offered by Clive Community Services’ Free Clinic.
